Few studies exist which look at psychological factors associated with physician sexual misconduct. In this study, we explore family dysfunction as a possible risk factor associated with physician sexual misconduct. Six hundred thirteen physicians referred to a continuing medical education (CME) course for sexual misconduct were administered the FACES-II survey, a validated and reliable measure of family dynamics. The survey was part of a self-learning activity. We collected data from February 2000 to February 2009. Participants were predominantly white, middle-aged males (...) who represented the full range of medical specialties. Their results were compared against a sample of 177 physicians. The FACES-II is a self-report test that measures family of origin (the family in which one was raised) dynamics on two dimensions (1) flexibility, ranging from too flexible (chaotic) to not flexible enough (rigid) and (2) cohesion ranging from too close (enmeshed) to not close enough (disengaged). The most common family pattern observed among physicians accused of sexual misconduct was rigid flexibility paired with disengaged cohesion, indicative of unhealthy family functioning. This pattern was significantly different than the pattern observed in the comparison group. Physicians who engage in sexual misconduct are more likely to have family of origin dysfunction. Ethics is developmental and learned in one’s family of origin. Family of origin dynamics may be one risk factor predisposing one to ethical violations. These findings have important implications for screening, education, and treatment across the medical education continuum. (shrink)

In Telling Silence, Charles E. Scott speaks of silence, often indirectly, in such ways as to create occasions in which people might become more aware of silence in their experiences of themselves and the world around them. The core question of the book is: how can people be aware of silence without turning it into a thing and losing it? Lack of awareness of silence is lack of awareness of a major dimension of lives, both human and nonhuman. (...) The purpose of the article is to offer a detailed exegetical analysis of the argument in chapter four of "de ente et essentia" in which aquinas argues for a distinction between "esse" and essence and to develop an interpretation of it on the basis of the analysis. I argue that the reconstructed argument shows that aquinas argues for a real distinction and that he establishes it earlier in the argument than some commentators have thought. Foucault's genealogies and archeologies provide occasions in which one may come to know the powers, accidents, and influences that have structured a particular knowledge or discipline. The Birth of the Clinic shows the development of modern medicine in a process by which rational inference and emphasis on the history of a disease are replaced by pathological anatomy. In modern anatomy, the corpse, not reason, became the “space” of modern medical knowledge. In this “space” developed a confederation of dead body, knowledge, (...) trained and noninferential gaze, organic unity, and the increased importance of bodily space instead of physical time. The discussion shows how Foucault's genealogy of modern medicine is influenced by and overcomes the knowledge that it describes. In this context, his genealogy is also shown to be self-overcoming and to raise important questions for medical ethics when ethical thought is conceived within the structures of knowledge that have been exposed by the genealogy. Why Fichte? I wondered, when the six-day Fichte Tagung was announced for Zwettl, Austria. Hegel and Kant and Hume had had their festivals. I expected that. But in spite of some party spirit for Fichte there is hardly a competitive passion on the part of Fichte people against Kant and Hegel and Hume people that would prompt a world congress. There is no International Society for the study of Fichte. There is indeed intense interest in his work, witnessed in the (...) new and carefully edited collection of his works, which is still in progress, the numerous, excellent and recently published studies, and the number of excellent Fichte articles appearing in leading journals. There is particular and growing interest in his revisions of his Wissenschaftslehre, in which his notion of intuition develops remarkably. His fully developed position does indeed offer a subtle and profound option to Hegel’s interpretation of consciousness. His influence on nineteenth century figures in literature as well as in philosophy is striking and important. But that has been so for a century and a half, and the same can be said of Schelling’s later work and influence, which is much less studied. (shrink)

The work of psychiatrist R. D. Laing deserves recognition as a key contribution to sociological theory, in dialogue with the interactionist and interpretivist sociological traditions. Laing encourages us to identify meaningful social action in what would otherwise appear to be nonsocial phenomena. His interpretation of schizophrenia as a rational strategy of withdrawal reminds us of the threat that others can pose to the self and how social relations are implicated in even the most "private" and "internal" of experiences. He developed (...) a far-reaching critical theory of the self in modern society, which challenges the medicalization and biochemical reduction of human problems. Using the case of shyness as an example, the article seeks to demonstrate the importance of Laing's theories for examining the fragility of the self in relation to contemporary social order. (shrink)
